In order to expedite your request for software installation please take a few moments to fill out as much of the requested information below as possible. Once you select “Submit”, a help desk ticket will automatically be generated on your behalf.
If the software or software version you wish to install has never been installed before then I&ITS will determine configuration, deployment and manageability requirements, perform a basic risk assessment and test the application before we set a date to install it on the workstation(s) noted. If the application shares student or patient information outside of the Faculty, then a formal Information Risk and Risk Assessment may be required. Before installation can happen, a report will be presented for an authorized signature. Program directors, the Director of clinics, managers, Vice Deans, Assistant Deans and the Dean are the only individuals permitted to authorize the installation of software once it has been assessed by I&ITS.
